After Hours
For medical emergencies, please call 911 or go to the nearest emergency department or urgent care facility.
For non-urgent issues please call during business hours or leave a message.
For urgent issues your call will be forwarded to an answering service.
Narcotic (Opioid) Medications
Chronic narcotic prescriptions will not be given to new patients.
Prescription Refills
Refill requests must be made 48 hours in advance to allow time for proper management and communication with the pharmacy. Refills after hours are discouraged.
Narcotics and controlled substance prescriptions will not be refilled after hours – no exceptions.
Cancellation and “No Show” Policy
Please be courteous and notify us right away if you are unable to make a scheduled appointment. This will allow time for others to fill that appointment time.
A “no show” is missing an appointment without notifying us, and it is an inconvenience to others and limits access to care. Repeated “no shows” may lead to termination from the practice.
Payment
We accept most insurance plans in Eastern Iowa, however each employer’s plan can be different, with different deductibles, co-pays, and co-insurance. It is the responsibility of the patient to provide us with accurate, updated insurance information at the time of each visit. Failure to do so may result in the patient being fully responsible for the visit.
Co-pays and co-insurance are expected at the time of service.
Statements for unpaid balances will be sent by mail or secure email. An unpaid balance for more than 2 months will incur a $20 late fee. For larger balances, a payment plan will be considered. If no arrangements are made, the account will be turned over to collections.
There will be a fee of $25 for returned checks.
Payment is accepted in person via cash, check, or credit card (Visa, MasterCard, or Discover). We also now accept Apple Pay and Google Pay.
